Protecting your family’s financial well-being in case of death, disability, or serious illness is an important part of planning for the future. As a Team Member, you’re automatically enrolled in Basic Life and AD&D insurance, giving you valuable coverage and peace of mind.
Supplemental Life and AD&D offer additional coverage at discounted rates. The chart describes the amounts of coverage you can buy for yourself, your spouse, and your child(ren).

For Supplemental Spouse Life: You may elect up to $50,000 for Supplemental Spouse Life Insurance without proof of good health (EOI).
For Team Members residing in most U.S. States, Unum STD coverage provides income replacement if you are sick or injured and need time off work to recover. The STD plan provides 60% of your weekly salary, to a maximum of $1,500 per week for up to 90 days of a disability. Active, full-time Team Members are automatically covered under the STD plan at no additional cost.
LTD benefits pick up where STD leaves off and are available to Team Members at discounted group rates. Your monthly benefit amount is calculated based on your pre-disability earnings (including regular wages and commissions), but does not include awards, bonuses, overtime pay, or other payments, up to $15,000. You have two options for LTD: a 60% income replacement plan and a lower-cost 50% income replacement plan.
You will need to provide Evidence of Insurability if you:
A condition will be considered pre-existing if, during the three months before your coverage starts, you received medical care, consultation, services (including diagnostic tests), or were prescribed medication. A disability will also be considered pre-existing if it begins within the first three months of coverage, unless you’ve gone 12 full months after your coverage starts without any treatment for that condition.
